In consideration of being allowed to participate in today’s clinic, the undersigned, on his or her own behalf, and/or on behalf of the participant(s) identified below, acknowledges, appreciates and agrees to the following conditions:
I, the parent/legal guardian of the participant(s), agree that the participant(s) and I shall comply with the stated and customary terms, rules and conditions for participation in the clinic. In addition, in the event of accident or injury when parent, legal guardian or emergency contact is not available, I give my permission to the FC Dallas clinic directors to procure medical attention.
I am aware that participation in a sports clinic with sports equipment creates a serious risk of injury, and I on behalf of myself and the participant(s), knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of others. I understand that by signing this Release and Waiver of Liability, I, for myself and the participant(s), and our respective heirs, assigns, administrators, personal representatives, and next of kin, hereby release and hold harmless, FC Dallas and the facility or organization hosting or sponsoring the clinic, including their affiliates, officers, members, agents, employees, other participants, from and against all claims, injuries, liabilities, or damages arising out of or related to participation in today’s clinic. I also acknowledge and agree that I will be responsible for any medical or other charges incurred in connection with the participant’s attendance at camp.
I give permission for photos of the participant to be taken and used on www.fcdallas.com.

I hereby acknowledge and agree to register my child with 6 Stones Mission Network, 209 N. Industrial Blvd., Suite 241, Bedford, Texas 76021 ("6 STONES") so that he/she may participate in the Liga HEB Intramural Soccer
Program (“Program”) and engage in certain activities related to the organization of and participation in afterschool soccer games and practices (collectively the “Play”) located in Tarrant County, Texas.
I am at least eighteen (18) years of age and legally competent to sign this Waiver of Liability and Disclaimer ("Waiver"). I hereby certify that, to the best of my knowledge, my child is in good health and able to participate in the Program and perform all Play associated with the Program. I also understand and agree that my child will not be paid for his/her performance, that he/she will not be covered by any medical or other insurance coverage by or through 6 STONES, and that he/she will not be entitled to any workers’ compensation benefits.
I understand that the Program and Play associated with the Program involve certain risks that are inherent in such activities, specifically including, but not limited to, property loss/damage, personal injury that may require certain first aid and/or medical treatment, and risks that I may not be able to foresee or anticipate.
In consideration of my child’s participation in the Program, I hereby acknowledge that I assume and accept all risks in connection with the Program, and Play associated with the Program, for any harm, injury, or damage that may befall me, my child, or my Property as a result of the Program, Play associated with the Program, and/or my child’s participation in the Program, including activities preliminary and subsequent to the Play and the Program, whether foreseen or unforeseen.
I understand and agree and hereby acknowledge that, except as otherwise stated herein, I will not attempt to hold 6 STONES or any of the Released Persons (as defined below) liable in any way for any occurrences arising out of the Program, Play associated with the Program, and/or my child’s participation in the Program that may result in injury, death, or other damages to me or my Property.
I DO HEREBY EXEMPT AND RELEASE 6 STONES, THE HURST EULESS BEDFORD INDEPENDENT SCHOOL DISTRICT AND THEIR RESPECTIVE OFFICERS, DIRECTORS, EMPLOYEES, VOLUNTEERS, CONTRACTORS, STAFF MEMBERS, AFFILIATES, AGENTS, ATTORNEYS, AND THE OWNERS, OCCUPIERS, AND TENANTS OF FACILITIES WHERE THE PLAY IS PERFORMED (COLLECTIVELY, THE “RELEASED PERSONS”) FROM ANY AND ALL LIABILITY WHATSOEVER FOR PERSONAL INJURY, PROPERTY DAMAGE, OR WRONGFUL DEATH CAUSED BY THE ACTS OR OMISSIONS OF ANY ONE OR MORE OF THE RELEASED PERSONS AND/OR ANY THIRD PARTIES ARISING OUT OF THE PROGRAM, WORK ASSOCIATED WITH THE PROGRAM, OR MY PARTICIPATION IN THE PROGRAM.
I FURTHER HEREBY ACKNOWLEDGE AND AGREE TO DEFEND, INDEMNIFY, SAVE, HOLD HARMLESS, AND COVENANT NOT TO SUE THE RELEASED PERSONS FOR ANY AND ALL CLAIMS, DEMANDS, DAMAGES, CAUSES OF ACTION AND SUITS IN EQUITY, WHETHER ARISING OUT OF COMMON LAW, EQUITY, ARBITRATION OR STATUTE, NOW OR HEREAFTER ARISING, KNOWN OR UNKNOWN, ASSERTED BY ME, MY CHILD, OR MY SPOUSE (AND MY OR THEIR RESPECTIVE ESTATES, HEIRS, EXECUTORS, ADMINISTRATORS, OR ASSIGNS) ARISING SOLELY OUT OF MY ACTS OR OMISSIONS THAT OCCURRED DURING THE PROGRAM, WORK ASSOCIATED WITH THE PROGRAM, OR MY PARTICIPATION IN THE PROGRAM.
I also hereby authorize, grant and convey unto 6 STONES all right, title, and interest in any and all photographic images and video or audio recordings made by 6 STONES during the Program and/or Play associated with the Program for the purpose of producing written, audio, or video material for publication in whatever form 6 STONES elects to publish such material in its sole discretion. In granting such permission, I hereby relinquish any right, title or interest in such photographic images, audio or video recordings, and all publications arising therefrom and grant 6 STONES permission to publish, exhibit, or use such materials for the purposes stated. In that regard, I hereby release 6 STONES and other staff, assigns and media from any suit, claim or demand arising out of, or occasioned by, the use of such material, specifically including, but not limited to, any royalties, proceeds, or other benefits derived from such photographs or recordings.
I hereby acknowledge and expressly agree that all indemnities, releases and waivers contained in this Waiver are intended to be as broad and inclusive as permitted by the laws of the State of Texas and that, if any portion of the agreements in this Waiver are held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect.
I understand the terms herein are contractual and not merely recitals, and that I have signed this document of my own free will.
I HAVE FULLY INFORMED MYSELF OF THE CONTENTS OF THIS WAIVER BY READING IT BEFORE I SIGNED IT.
